FOTA AWARDS

The Louise Samson Leadership Award:

This award is in recognition of leadership that has contributed to the advancement of occupational therapy in Florida.

Criteria: Occupational Therapists and/or Occupational Therapy Assistants, FOTA members in good standing 

The OT David D Clark Award of Excellence:

This award is for longstanding and significant contribution to the profession of occupational therapy and FOTA in Florida.

Criteria: Occupational Therapists, FOTA members in good standing 

The OTA David D Clark Award of Excellence:

This award is for longstanding and significant contribution to the profession of occupational therapy and FOTA in Florida.

Criteria: Occupational Therapist Assistants, FOTA members in good standing

FOTA Award of Recognition:

This award is for outstanding contribution to the recognition and advancement of occupational therapy in Florida. There can be more than one recipient for this award.

Criteria: Occupational Therapists and/or Occupational Therapy Assistants, FOTA members in good standing 

FOTA Legislator of the Year:

This award is for significant contribution to the recognition and advancement of occupational therapy in Florida.

Criteria: Occupational Therapists and/or Occupational Therapy Assistants, FOTA members in good standing 

FOTA Award of Appreciation:

This award is to other persons and/or organizations that have rendered special services to FOTA or the occupational therapy profession in Florida. There can be more than one recipient for this award.

Criteria: Cannot be Occupational Therapists and/or Occupational Therapy Assistants

The Call for Nominations for the 2010 Annual FOTA Awards and Recognition Program is open!!!

FOTA encourages you to recognize individuals that have made outstanding contributions to the occupational therapy profession in Florida.

Awards will be presented at the FOTA Annual Membership Meeting held during the AOTA Convention and Expo in Orlando  (FOTA Membership meeting is April 30, 2010 @ the Hilton 6:30-8pm). Application for nominations will be accepted only via electronic submission.
